Payment Plan Request for New Members
By completing this form you agree to the following terms and conditions.
I/We authorise and request BNI Melbourne East ABN: 29 100 495 332 to debit payments from our Bank Account as specified below, at intervals and amounts as stated on this application and as per the terms and conditions of this agreement.
If this application is approved, I undertake to complete a direct debit payment authority through InteraPay on behalf of BNI Melbourne East.
1st Payment of BNI New Member Registration and Training Fee $495 (inc GST) plus the Payment Plan Administration Fee $55 (inc GST) on approval of this Payment Plan: $550 (inc GST)
2nd Payment of the 15th of the month following the 1st payment: $272.25 (incl GST)
3rd Payment of the 15th of the month following the 2nd payment (or if the 15th falls on a weekend, the next business day): $272.25 (incl GST)
4th Payment 1 month after 3rd payment: $272.25 (incl GST)
5th Payment 1 month after 4th Payment: $272.25 (incl GST)
Total being for 12 month Membership fee payment of $1089 (incl GST) plus $495 (incl GST) Registration fee plus $55 (incl GST) for a total of $1629 (incl GST)
I understand that there is a $22 (incl GST) fee for any dishonoured payments, which will be payable on the next Monday (further payment schedules remain the same).
Please note this facility is at the absolute discretion of the Executive Director of the region. Members with default history will not be offered this facility.
I understand that if I default on this payment plan, I will be removed from BNI Connect without a certificate of credit and not permitted to continue on the BNI program.

Members defaulting on this plan, will be dropped from BNI Connect without a certificate of credit and not allowed to continue in the program.
